06. Proofs Needed For Social Security Applications Death Benefits * Retirement Benefits

-- Your Social Security card (or a record showing the number).

__ Spouse's Social Security card (or a record showing the number).

__ Proof of your age (birth certificate, etc.).

__ Your Marriage Certificate.

__ Birth Certificate for each child entitled to benefits.

__ Certified copy of Adoption Order (if child is adopted).

__ Death Certificate of worker who died.

__ Divorce papers (where applicable).

__ Copy of your latest income-tax "withholding statement" (Form W-2). Or,

if you are self-employed, copy of your last income-tax return.

__ Receipted funeral bill if someone other than the widow or widower
claims lump-sum death benefit.

You should secure all available documents NOW - while they are easier to obtain and before they are needed. If you become disabled, it is a good idea to keep a list of all doctors who have treated you, as well as all hospitals,
clinics and other institutions.

Application for benefits should be made at your nearest local field office of the Social Security Administration.
